Commodity Category the applicant wishes to deal in;
Business License of the applicant issued by the Administration Bureau of Industry and Commerce;
Feasibility Study Report of the Application (applicable only when the applicant is a Wholesale or Retail Enterprise);
Balance Sheet and Income Sheet of the last two years (applicable only when the applicant is a Wholesale or Retail Enterprise).
